WHEREAS, the City of Newark Department of Health and Community Wellness/Mary Eliza Mahoney Health Centers requires a Medical Director, to assist in the organization and assignment of medical staff, monitoring the activities of the medical staff to ensure that productivity and quality levels are met and management of the Health Centers; and
WHEREAS, on or about March 19, 2014, through Resolution 7R2-c, the Municipal Council authorized the Mayor and/or the Director of Child and Family Well-Being now known as Department of Health and Community Wellness entered into a contract with Forest Hill Family Health Associates, PA to provide Thomas R. Ortiz, M.D. as a Medical Director Consultant in an amount not to exceed $162,000.00, for the term to commence upon adoption and not to exceed one year; and
