WHEREAS, the City of Newark's Department of Health and Community Wellness (hereinafter, "DHCW") provides medical care services, pursuant to and consistent with the Health Center Program Laws and Regulations (including Section 330 of Public Health Services Act, 42 U.S.C. Section 254b and 42 C.F.R. ? 51c.301, et seq.); and

WHEREAS, the United States Department of Health and Human Services/Health Resources and Services Administration has made grant funds available for the provision of these services; and

WHEREAS, on June 24, 2021, on behalf of the City of Newark, the Director of the DHCW submitted an application to the United States Department of Health and Human Services/Health Resources and Services Administration (hereinafter, "HRSA"), Hubert H.
