WHEREAS, the City of Newark provides community outreach, education and medical services related to sexually transmitted diseases, including but not limited to, high quality medical and laboratory services to residents of the City of Newark at the City’s Communicable Disease Prevention and Treatment Center (CDPTC); and

 

WHEREAS, the New Jersey Department of Health and Senior Services (hereinafter “NJDOH”) has made grant funds available for the provision of such services at the CDPTC; and

 

WHEREAS, on May 16, 2017, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness (hereinafter “DHCW”) submitted an application to the NJDOH, P.O. Box 360, Trenton, New Jersey 08625, requesting grant funding in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), for the continued provision of community outreach, education and medical services related to sexually transmitted diseases for the period commencing on July 1, 2017, and terminating on June 30, 2018 (a copy of the Grant Application is attached hereto and made a part hereof); and

 

WHEREAS, as a result of this application, the City of Newark was awarded a grant in the amount of eighty-nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), from the NJDOH for the period commencing on July 1, 2017, and terminating on June 30, 2018 (a copy of the Notice of Grant Award is attached hereto and made a part hereof); and

 

WHEREAS, the City of Newark, DHCW, desires to accept the awarded grant funds in the amount of eighty-nine thousand, one hundred thirteen dollars and zero cents ($89,113.00); and

 

WHEREAS, the City of Newark recognizes that the NJDOH may extend the time of the grant award and/or provide additional funds if made available; and

 

WHEREAS, in such cases, the NJDOH will amend the grant to reflect the grant time extension and/or increase or decrease of grant funds as applicable; and

 

WHEREAS, the terms of this grant award may only be waived or amended by the parties duly authorized to execute the grant, subject to the approval of the Newark Municipal Council by resolution.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor and/or his designee, the Director of the Department of  Health and Community Wellness, are hereby authorized to submit an application to the  New Jersey Department of Health and Senior Services, P.O. Box 360, Trenton, New Jersey 08625, requesting funds in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), in order to provide community outreach, education and medical services related to sexually transmitted diseases, including, but not limited to, high quality medical and laboratory services, to residents of the City of Newark at the City’s Communicable Disease Prevention and Treatment Center for the period commencing on July 1, 2017, and terminating on June 30, 2018.

 

2.                     The Municipal Council hereby ratifies its authorization of this application from the date of the application’s submission to the date of adoption of this authorizing resolution.

 

3.                     The grant award in the amount of eighty-nine thousand, one hundred thirteen dollars and zero cents ($89,113.00) from the New Jersey Department of Health and Senior Services, is hereby accepted.

 

4.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to receive and expend the grant award for the continued provision of community outreach, education and medical services related to sexually transmitted diseases for the period commencing on July 1, 2017, and terminating on June 30, 2018.

 

 

5.                     The Municipal Council hereby ratifies the acceptance of this grant award from July 1, 2017, to the date of adoption of this authorizing resolution.

 

6.                     No municipal funds are required for receipt or acceptance of this grant award.

 

7.                     The Director of the Department of Health and Community Wellness shall file a duly executed copy of the grant application and the grant award letter in the Office of the City Clerk.           

 

STATEMENT

 

This resolution ratifies and authorizes the actions taken by the Mayor and/or his designee, the Director of the Department of Health and Community Wellness, to apply for and accept grant funds in the amount of Eighty Nine Thousand, One Hundred Thirteen Dollars and Zero Cents ($89,113.00), from the New Jersey Department of Health and Senior Services, to provide community outreach, education and medical services related to sexually transmitted diseases for the period commencing on July 1, 2017, and terminating on June 30, 2018, at the City’s Communicable Disease Prevention and Treatment Center. No municipal funds are required for the acceptance of the grant funds.
