WHEREAS, the Department of Health and Community Wellness provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and
WHEREAS, the New Jersey Department of Health (NJDOH) has made funds available for the provision of such services through the Immunization 2020 Grant; and
WHEREAS, on May 17, 2019, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness, submitted an electronic application to the NJDOH requesting grant funding in the amount of $522,000.00 from the Vaccine Preventable Disease Program to continue support to increase immunization levels; and
WHEREAS, as a result of the application, the City of Newark has been awarded an initial grant in the amount of $190,000.00 from the Vaccine Preventable Disease Program to continue support to increase immunization levels for the period commencing on July 1, 2019 and terminating on June 30, 2020; and
WHEREAS, the City of Newark recognizes that NJDOH may extend the time of the grant award and/or provide additional funds, if made available; and
WHEREAS, NJDOH will amend the grant to reflect the extension of the grant period and/or increase or decrease of funds.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are authorized to submit an electronic application to NJDOH requesting grant funding in the amount of $522,000.00 from the Vaccine Preventable Disease Program to continue support to increase immunization levels (a copy of the application is attached hereto and made a part hereof).
2. The Municipal Council ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.
3. The initial grant award in the amount of $190,000.00 from NJDOH is accepted (a copy of the letter of intent is attached hereto and made a part hereof).
4. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are authorized to enter into and execute grant documents with NJDOH to receive and expend the grant award in the amount of $190,000.00 to continue support to increase immunization levels for the period commencing on July 1, 2019 and terminating on June 30, 2020.
5. The Municipal Council ratifies the acceptance of this grant award from July 1, 2019 to the date of adoption of this authorizing resolution.
6. No municipal funds are required for receipt or acceptance of this grant award.
7. The Director of the Department of Health and Community Wellness shall file a copy of the grant application, letter of intent and all supporting documents with the Office of the City Clerk.
STATEMENT
This resolution ratifies and authorizes the actions taken by the Director of the Department of Health and Community Wellness, to submit an electronic application for grant funding from the New Jersey Department of Health (NJDOH), to continue support to increase immunization levels; and furthermore, ratifies and authorizes the Mayor and/or his designee, the Director of the Department of Health and Community Wellness, to accept initial grant funds in the amount of $190,000.00 from NJDOH to continue support to increase immunization levels for the period commencing on July 1, 2019 and terminating on June 30, 2020. No municipal funds are required for receipt and acceptance of this grant award.
