WHEREAS, the Department of Health and Community Wellness (“DHCW”) provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark (the “City”); and

WHEREAS, the State of New Jersey Department of Health, P.O. Box 369, Trenton, New Jersey 08625 (“NJDOH”) through its Vaccine Preventable Disease Program has made funds available for the provision of such services; and

WHEREAS, on or about May 28, 2026, on behalf of the City,, the Director of the DHCW, submitted an application to the State of New Jersey Department of Health, P.O. Box 369, Trenton, New Jersey 08625, requesting grant funding in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), for the continued provision of Immunization Project Vaccinate Program for immunization service delivery, vaccine storage and handling, community outreach and case management for children (a copy of the Application is attached here to and made a part hereof); and

WHEREAS, , on July 09, 2026, the NJDOH sent the City a Letter of Intent to Fund (“LOI”) informing of the intent to award grant funds in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents($388,000.00), from the , for the continued provision of Immunization Project Vaccinate Program for immunization service delivery, vaccine storage and handling and community outreach for children and adults, for the period commencing on July 1, 2026 and terminating on June 30, 2027 (a copy of the Revised Letter of Intent to Fund is attached hereto and made a part hereof); and

WHEREAS, the City’s DHCW, desires to accept the Grant Funds in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00); and

WHEREAS, the City recognizes that the NJDOH may extend the time of the Grant Award and/or provide additional funds, if made available; and

WHEREAS, the City’s Department of Health and Community Wellness shall, upon any extension of the Grant Period and or increase or decrease of additional funds, file an amending Resolution reflecting the extension of time and award amount of the Grant.


NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

The Mayor of the City of Newark and/or his designee, the Director of the Department of Health and Community Wellness (“DHCW”), are hereby authorized to submit an application to the State of New Jersey, Department of Health, P.O. Box 369, Trenton, New Jersey 08625, requesting grant funding in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), for the continued provision of Immunization Action Plan/Project Vaccinate Program for immunization service delivery, vaccine storage and handling, and community outreach for children.

The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing Resolution.

The Grant Award in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), from the NJDOH is hereby accepted.

The Mayor of the City of Newark and/or his designee, the Director of the DHCW, are hereby authorized to enter into and execute grant documents with the NJDOH, to receive and expend the grant award, Immunization Action Plan/Project Vaccinate Program, for the period commencing on July 1, 2026 and terminating on June 30, 2027.

The Municipal Council hereby ratifies the acceptance of this Grant Award from July 1, 2026, to the date of adoption of this authorizing Resolution.

The City’s DHCW shall, upon any extension of time of the grant and or increase or decrease of additional funds, file an amending resolution reflecting the extension time and award amount of the grant.

No municipal funds are required for receipt or acceptance of this Grant Award.

The Director of the DHCW shall file a fully executed copy of the grant application and the Letter of Intent to Fund in the Office of the City Clerk.


STATEMENT

This Resolution ratifies and authorizes the actions taken by the Mayor of the City of Newark and or/his designee, the Director of the Department of Health and Community Wellness, to apply for and accept a grant award in the amount of $388,000.00, from the State of New Jersey, Department of Health, P.O. Box 369, Trenton, New Jersey 08625, for the continued provision of Immunization/Project Vaccinate Program for immunization service delivery, vaccine storage and handling, and community outreach for the period commencing on July 1, 2026 and terminating on June 30, 2027. No municipal funds are required for receipt or acceptance of the grant award.
