WHEREAS, Greater Newark Health Care Coalition Inc. (“GNHCC”), 80 Main Street, Suite 210, West Orange NJ 07052, has been awarded the multi-year Grant from New Jersey State Department of Health; and

 

WHEREAS, the New Jersey State Department of Health approved released carryover funds leading to an award to the City of Newark (the “City”); and

 

WHEREAS, GNHCC has allocated Grant Funds to the City, for the implementation of Hypertensive quality project for the Newark Learning Collaborative in the amount of One Hundred Thousand Dollars and Zero Cents ($100,000.00), for the Grant Period of July 1, 2025 through September 29, 2026; and

 

WHEREAS, the Director of the Department of Health and Community Wellness (“DHCW”) desires to accept said grant funds; and

 

WHEREAS, the City recognizes that GNHCC may extend the period of the grant award and/or provide additional funds when it becomes available; and

 

WHEREAS, GNHCC may amend the grant award to reflect the extension of the grant period and/or increase or decrease the amount of the award; as appropriate and which shall be subject to approval by Municipal Council by resolution; and

 

WHEREAS, the DHCW, shall upon any such extension of time of the grant and/or increase/decrease of additional funds, file an amending resolution reflecting the final term and award amount of the grant; and 

 

WHEREAS, terms of this grant award may only be waived or amended by the parties duly authorized to execute the grant, subject to the approval of the Municipal Council by resolution; and

 

WHEREAS, no municipal funds are required for receipt of acceptance of these grant funds.

 

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor and/or his designee, and the Director of the Department of Health and Community Wellness, are hereby authorized to accept and expend the grant funds from Greater Newark Health Care Coalition Inc., 80 Main Street, Suite 210, West Orange, New Jersey 07052, in the amount of One Hundred Thousand Dollars and Zero Cents ($100,000.00) for advocacy support in implementation of Hypertensive quality project for the Newark Learning Collaborative for the Grant Period commencing on July 1, 2025 and terminating September 29, 2026.

 

2.                     Municipal Council hereby ratifies its authorization for the acceptance of additional grant funds for the grant period of July 1, 2025 through September 29, 2026.

 

3.                     The Municipal Council hereby ratifies its authorization of this grant acceptance from the July 1, 2025 to the date of adoption of this authorizing Resolution.

 

4.                     The Grant Award in the amount of One Hundred Thousand Dollars and Zero Cents ($100,000.00), from Greater Newark Health Care Coalition Inc. is hereby accepted (a copy of the Grant Agreement is attached hereto and made a part hereof).   

 

5.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to file the electronic acceptance necessary to accept the grant award.

 

6.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to receive and expend the grant award for the advocacy support in  implementation of Hypertensive quality project for the Newark Learning Collaborative.

 

7.                     The Director of the DHCW shall file a fully executed copy of the Grant Agreement in the Office of the City Clerk.

 

8.                      No municipal matching funds are required for the receipt or acceptance of this grant award.

 

 

STATEMENT

 

 

This Resolution authorizes and ratifies the actions taken by the Mayor and/or his designee, the Director of Health and Community Wellness, to accept and expend grant funds in the amount of One Hundred Thousand Dollars and Zero Cents ($100,000.00) from the Greater Newark Health Care Coalition Inc., 80 Main Street, Suite 210, West Orange, New Jersey 07052, for advocacy support in the implementation of Hypertensive quality project, for the period commencing on July 1, 2025 and terminating September 29, 2026. No municipal matching funds are required for the receipt or acceptance of these grant funds.
