 Program designed to reduce the number and the seriousness of injuries for seniors who fall.

 WHEREAS, the New Jersey Department of Human Services, Division of Aging Services (DoAS) announced the availability of Falls Prevention grant program funds to increase the number of older adults and/or adults with disabilities at risk of falling who successfully complete Falls Prevention Programs; and

 

WHEREAS, the City of Newark has a need for said grant funding to train master trainers and coaches to educate caregivers of seniors and adults with disabilities in Fall Prevention Programs; and

 

WHEREAS, the City of Newark desires to further the public interest by obtaining funding from the DoAS Falls Prevention grant program; and

 

WHEREAS, on or about November 12, 2015, the Director of the Department of Health and Community Wellness (DHCW), on behalf of the City of Newark, submitted an application to DoAS requesting Falls Prevention Programs funds in the amount of $12,000.00 (a copy of said application is attached hereto and made a part hereof); and

 

                     WHEREAS, pursuant to the terms of the DoAS Request for Proposals for Falls Prevention Programs, a copy of which is attached hereto and made a part hereof, the City of Newark is required to submit a resolution from the Newark Municipal Council authorizing submission of the grant application; and

 

                     WHEREAS, upon notification of the receipt of a Falls Prevention Program grant award in connection with this application, the Director of DHCW desires to accept the grant award and execute a grant agreement governing said award; and

 

                     WHEREAS, upon notification of the receipt of a grant award in connection with this application, the Director of DHCW shall submit a copy of the grant agreement to the Corporation Counsel and the Business Administrator for review, approval, and signature.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness (DHCW), are hereby authorized to submit an application to the Department of Aging Services (DoAS) requesting Falls Prevention Program grant funds in the amount of $12,000.00 for training master trainers and coaches to educate caregivers for seniors and adults with disabilities in the Falls Prevention Programs.

 

2.                     The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.

 

3.                     Upon notification of the receipt of a grant award in connection with this application, the Falls Prevention Program grant funds are hereby accepted.

 

4.                     Upon notification of the receipt of a Falls Prevention Program grant award in connection with this application, the Mayor and/or his designee, the Director of DHCW, are hereby authorized to enter into and execute grant documents with DoAS to receive and expend the Falls Prevention Program grant award for training master trainers and coaches to educate caregivers for seniors and adults with disabilities in the Falls Prevention Programs.

 

5.                     Upon notification of the receipt of a Falls Prevention Program grant award in connection with this application, the Director of DHCW shall submit a complete copy of the grant agreement and any associated documents to both the Corporation Counsel and the Business Administrator for review, approval, and signature.

 

6                     No municipal funds are required for receipt or acceptance of this grant award.

 

7.                     The Director of DHCW shall file a fully-executed copy of the grant application, the grant award letter, and the grant agreement in the Office of the City Clerk.

 

STATEMENT

 

This resolution ratifies and authorizes the Mayor and/or his designee, the Director of the Department of Health and Community Wellness, to submit an application for, accept, and enter in an agreement for Falls Prevention Programs grant funding in the amount of $12,000.00 from the New Jersey Department of Human Services, Division of Aging Services, for training master trainers and coaches to educate seniors caregivers and adults with disabilities in the Falls Prevention Programs.
