WHEREAS, the Department of Health and Community Wellness, provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and
 
WHEREAS, the State of New Jersey Department of Health-Vaccine Preventable Disease Program, has made funds available for the provision of such services; and
 
      WHEREAS, on December 2, 2013, on behalf of the City of Newark, the Director of Health and Community Wellness submitted an application to the State of New Jersey Department of Health -Vaccine Preventable Disease Program, P.O. Box 369, Trenton, New Jersey 08625, requesting grant funding in the amount of two hundred and fifty thousand dollars ($250,000.00), for the continued provision of Immunization /Project Vaccinate Program for immunization service delivery, vaccine storage and handling, community outreach and case management for children; and
 
WHEREAS, as a result of this application, the City of Newark has been awarded a grant in the amount of two hundred seventy-six thousand, two hundred fifty dollars and no cents ($276,250.00), from the State of New Jersey, Department of Health-Vaccine Preventable Disease Program, for the continued provision of Immunization /Project Vaccinate Program for immunization service delivery, vaccine storage and handling and community outreach for children and adults, for the period commencing on January 1, 2014, and terminating on December 31, 2014; and
 
WHEREAS, the City of Newark, Department of Health and Community Wellness desires to accept funds in the amount of  two hundred seventy-six thousand, two hundred fifty dollars and no cents ($276,250.00); and
 
WHEREAS, the City of Newark recognizes that the State of New Jersey, Department of Health-Vaccine Preventable Disease Program may extend the time of the Grant Award and/or provide additional funds, if made available; and
 
WHEREAS, the State Department of Health -Vaccine Preventable Disease Program will amend the grant to reflect the grant time extension and/or increase or decrease of additional funds if made available; and
 
WHEREAS, the City of Newark wishes to agree to any amendment to the Grant.
 
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
    1. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness are hereby authorized to submit an application to the State of New Jersey, Department of Health-Vaccine Preventable Disease Program, P.O. Box 469, Trenton, New Jersey 08625, requesting grant funding in the amount of  two hundred and fifty thousand dollars ($250,000.00), for the continued provision of Immunization Action Plan /Project Vaccinate Program for immunization service delivery, vaccine storage and handling, and community outreach for children, no City funds required (a copy of the application is attached hereto and made a part hereof).
    2. The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.
    1. The grant award in the amount of two hundred seventy-six thousand, two hundred fifty dollars and no cents ($276,250.00), from the State of New Jersey, Department of Health-Vaccine Preventable Disease Program is hereby accepted (a copy of the grant award letter is attached hereto and made a part hereof).
    1. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to enter into and execute grant documents with the State of New Jersey, Department of Health-Vaccine Preventable Disease Program, to receive and expend the grant award in the amount of two hundred seventy-six thousand, two hundred fifty dollars and no cents ($276,250.00), for the continued provision of immunization service delivery, vaccine storage and handling, and community outreach for children, for the period commencing on January 1, 2014, and terminating on December 31, 2014 (a copy of the grant award is attached hereto and made a part hereof).
    1. The Municipal Council hereby ratifies the acceptance of this grant award from January 1, 2014 to the date of adoption of this authorizing resolution.
    1. No municipal funds are required for receipt or acceptance of this grant award.
    1. The Director of Department of Health and Community Wellness shall file a fully executed copy of the grant application and grant award letter in the Office of City Clerk.
 
 
STATEMENT
 
 
This resolution ratifies and authorizes the actions taken by the Mayor and or/his designee, the Director of Department of Health and Community Wellness, to apply for and accept the grant award  in the amount of two hundred seventy-six thousand, two hundred fifty dollars and no cents ($276,250.00) from the State of New Jersey, Department of Health-Vaccine Preventable Disease Program, P.O. Box 469, Trenton, NJ 08625, for the continued provision of Immunization /Project Vaccinate Program for immunization service delivery, vaccine storage and handling, and community outreach for the period commencing on January 1, 2014, and terminating on December 31, 2014. No municipal funds are required from the municipal budget.
