WHEREAS, Greater Newark Health Care Coalition Inc. (“GNHCC”), 80 Main Street, Suite 210, West Orange, New Jersey 07052, has been awarded the multi-year Grant from the New Jersey State Department of Health (“NJDOH”), Division of Community Health Services, Community Health and Wellness Unit (“CHWU”), and desires to collaborate with the City to address the effects of unmet essential needs on cardiovascular outcomes by providing data management support in the implementation of Hypertensive quality project; and
WHEREAS, GNHCC released allotted grant funds in the amount of Fifteen Thousand Dollars and Zero Cents ($15,000.00) for the Period of July 1, 2025 through June 30, 2026 to the City; and
WHEREAS, the Director of the DHCW desires to accept said Grant Funds; and
WHEREAS, the City recognizes that GNHCC may amend the grant to reflect an extension of the grant period and/or increase or decrease the amount of the grant award as appropriate and which shall be subject to approval by Municipal Council by resolution; and
WHEREAS, the City’s DHCW shall, upon any such extension of time of the grant and/or increase/decrease of additional grant funds, file an amending resolution reflecting the final term and award amount of the grant; and
WHEREAS, the terms of this Grant Award may only be waived or amended by the parties duly authorized to execute the grant, and subject to the approval of the Municipal Council by resolution; and
WHEREAS, no municipal funds are required for receipt of acceptance of these grant funds.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Mayor and/or his designee, and the Director of the Department of Health and Community Wellness (“DHCW”), are hereby authorized to accept and expend the funds from Greater Newark Health Care Coalition, Inc., 80 Main Street, Suite 210, West Orange, New Jersey 07052 (“GNHCC”), in the amount of Fifteen Thousand Dollars and Zero Cents ($15,000.00), for the Grant Period of July 1, 2025 through June 30, 2026 for data management support in the implementation of Hypertensive quality project (a copy of the agreement is attached hereto and made a part hereof).
2. The Municipal Council hereby ratifies its authorization of this grant funds acceptance from the date of acceptance of funds submission to the date of adoption of this authorizing resolution.
3. The Grant Award in the amount of Fifteen Thousand Dollars and Zero Cents ($15,000.00), from the GNHCC is hereby accepted (a copy of the agreement is attached hereto and made a part hereof).
4. The Director of the Mayor’s Office of Homeless Services shall file a fully executed copy of the grant agreement and the MOUs in the Office of the City Clerk.
5. The Newark Municipal Council hereby ratifies its authorization of the Grant Agreement from April 30, 2026 to the date of adoption of this authorizing Resolution.
6. The Director of the DHCW shall file a fully executed copy of the Grant Agreement in the Office of the City Clerk.
7. No municipal funds are required for receipt of acceptance of these grant funds.
STATEMENT
This Resolution ratifies and authorizes the actions taken by the Mayor of the City of Newark and/or his designee, the Director of Health and Community Wellness, to accept and expend grant funds in the amount of $15,000.00 from Greater Newark Health Care Coalition, Inc., 80 Main Street, Suite 10, West Orange, New Jersey 07052, for data management support in the implementation of Hypertensive quality project, for a period commencing July 1, 2025 through June 30, 2026. No municipal funds are required for the receipt or acceptance of this Grant Award.
