WHEREAS, Kiyata I. Derrick (Ms. Derrick) has been an employee of the City of Newark, (the City) as a Police Officer from April 26, 2005, through the present as a Police Officer; and

 

WHEREAS, Ms. Derrick is an active member of the Police and Firemen’s Retirement System (PFRS) and has over sixteen (16) years of PFRS Service Credit, meeting the minimum service requirement of at least four (4) years of PFRS membership; and

 

WHEREAS, the Director of the Department of Public Safety, based upon the advice and documentation provided by physicians, certifies that Ms. Derrick is unable to perform her assigned duties; and

 

WHEREAS, the City is unable to provide an alternative to the PFRS covered position with duties capable of being performed by Ms. Derrick; and

 

WHEREAS, the Public Safety Director has consented to the processing of the Involuntary Disability Retirement application.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.  The Business Administrator, the Director of Public Safety and the Director of Personnel are authorized to process the Involuntary Disability Retirement application for Ms. Derrick.

 

2.  Certified copies of this Resolution shall be forwarded to all parties involved in the administration of this action.

Resolution authorizing the processing of an application of the Involuntary Disability Retirement of Police Officer, Kiyata I. Derrick, an employee of the City of Newark, Department of Public Safety, Police Division.
