WHEREAS, the Department of Health and Community Wellness provides enhance adult influenza vaccination coverage through the FQHC Influenza Vaccination Initiative 2021 in the City of Newark; and
WHEREAS, the State of New Jersey Department of Health, Division of Community Health Services has made funds available for the provision of such services; and
WHEREAS, on August 14, 2020, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness, submitted an application to the State of New Jersey Department of Health, Division of Community Health Services, P.O. Box 364, Trenton, New Jersey 08625-0364, requesting grant funding in the amount of Four Hundred Eighty-Seven Thousand Three Hundred Ninety-Three Dollars and Ninety-Eight Cents ($487,393.98), for the provision to enhance adult influenza vaccination coverage (a copy of the application is attached here to and made a part hereof); and
WHEREAS, as a result of this application, the City of Newark has been awarded a grant in the amount of Four Hundred Eighty-Seven Thousand Three Hundred Ninety-Four Dollars and Zero Cents ($487,394.00), from the State of New Jersey Department of Health, Division of Community Health Services, P.O. Box 364, Trenton, New Jersey 08625-0364, for the provision to enhance adult influenza vaccination coverage, for the period commencing on October 1, 2020, and terminating on June 30, 2021 (a copy of the grant award letter is attached hereto and made a part hereof); and
WHEREAS, the City's Department of Health and Community Wellness, desires to accept funds in the amount of Four Hundred Eighty-Seven Thousand Three Hundred Ninety-Four Dollars and Zero Cents ($487,394.00); and
WHEREAS, the City of Newark recognizes that the State of New Jersey Department of Health, Division of Community Health Services, may extend the time of the Grant Award and/or provide additional funds, if made available; and
WHEREAS, the State of New Jersey Department of Health, Division of Community Health Services, will amend the grant to reflect the grant time extension and/or increase or decrease of additional funds, if made available.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness are hereby authorized to submit an application to the State of New Jersey Department of Health, Division of Community Health Services, P.O. Box 364, Trenton, New Jersey 08625, requesting grant funding in the amount Four Hundred Eighty-Seven Thousand Three Hundred Ninety-Three Dollars and Ninety-Eight Cents ($487,393.98), for the provision to enhance adult influenza vaccination coverage.
2. The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.
3. The grant award in the amount of Four Hundred Eighty-Seven Thousand Three Hundred Ninety-Four Dollars and Zero Cents ($487,394.00), from the State of New Jersey Department of Health, Division of Community Health Services is hereby accepted.
4. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness are hereby authorized to enter into and execute grant documents with the State of New Jersey Department of Health, Division of Community Health Services, to receive and expend the grant award in the amount of Four Hundred Eighty-Seven Thousand Three Hundred Ninety-Four Dollars and Zero Cents ($487,394.00), for the provision to enhance adult influenza vaccination coverage, for the period commencing on October 1, 2020 and terminating on June 30, 2021.
5. The Municipal Council hereby ratifies the acceptance of this grant award from October 1, 2020, to the date of adoption of this authorizing resolution.
6. The City of Newark, Department of Health and Community Wellness shall, upon any extension of time of the grant and or increase or decrease of additional funds, file an amending fiscal resolution reflecting the final time and award amount of the grant.
7. No municipal funds are required for receipt or acceptance of this grant award.
8. The Director of the Department of Health and Community Wellness shall file a fully executed copy of the grant application and grant award letter in the Office of the City Clerk.
STATEMENT
This resolution ratifies and authorizes the actions taken by the Mayor and or/his designee, the Director of the Department of Health and Community Wellness, to apply for and accept a grant award in the amount of $487,394.00, from the State of New Jersey Department of Health, Division of Community Health Services, P.O. Box 364, Trenton, New Jersey 08625, for the provision to enhance adult influenza vaccination coverage, for the period commencing on October 1, 2020 and terminating on June 30, 2021. No municipal funds are required for receipt or acceptance of the grant award.
