WHEREAS, the Department of Child and Family Well-Being/Homeless Health Care Program, continues to provide medical care, social services, substance abuse and mental health counseling to the homeless population of the City of Newark; and
 
WHEREAS, the Department of Child and Family Well-Being/Homeless Health Care Program seeks to increase its hours of operation and enhanced access to medical care, social services, substance abuse and mental health counseling to the homeless population for the City of Newark; and
 
WHEREAS, the Department of Health and Human Services/Health Resources and Services Administration (HRSA), has made funds available for the provision of such services; and
 
WHEREAS, the Department of Health and Human Services/Health Resources and Services Administration (HRSA), has made supplemental funds available under the Patient-Centered Medical Home Initiative to increase hours of operation and enhance access to medical care; and
 
      WHEREAS, on July 21, 2011, on behalf of the City of Newark, the then Acting Director of the Department of Child and Family Well-Being submitted an application to the Department of Health and Human Services/Health Resources and Services Administration (HRSA), Hubert H. Humphrey Building, 200 Independence Avenue, S.W., Washington, DC 20201, requesting a grant in the amount of $1,606,073.00, to provide health care, social services, substance abuse and mental health services to the homeless population of the City of Newark; and
 
WHEREAS, as a result of this application, the City of Newark has been awarded a grant in the amount of $669,177.00 from the Department of Health and Human Services/Health Resources and Service Administration (HRSA) for the continued provision of health care, social services, substance abuse and mental health services to the homeless population of the City of Newark, for the period commencing on November 1, 2011 and terminating on October 31, 2012; and
 
      WHEREAS, on September 2, 2011, on behalf of the City of Newark, the then Acting Director of the Department of Child and Family Well-Being submitted an application to the Department of Health and Human Services/Health Resources and Services Administration (HRSA), Hubert H. Humphrey Building, 200 Independence Avenue, S.W., Washington, DC 20201, requesting a grant in the amount of $35,000.00, to enhance access to medical care and increase the number of hours that continued said services are provided to the homeless population of the City of Newark; and
 
 WHEREAS, as a result of this application, the City of Newark has been awarded a grant in the amount of $35,000.00 from the Department of Health and Human Services/Health Resources and Service Administration (HRSA), to enhance access to medical care and increase the number of hours that continued services are provided to the homeless population of the City of Newark, for the period commencing on November 1, 2011 and terminating on October 31, 2012; and
 
WHEREAS, the Department of Health and Human Services/Health Resources and Services Administration (HRSA), does not require the City to execute a grant agreement in order to accept the grant award; and
 
WHEREAS, the City of Newark recognizes that the Department of Health and Human Services/Health Resources and Services Administration(HRSA), may extend the time of the Grant Award and/or provide additional funds If made available; and
 
WHEREAS, the Department of Health and Human Services/Health Resources and Services Administration (HRSA), will amend the grant(s) to reflect the grant time extension and/or increase or decrease of additional funds if made available; and
 
WHEREAS, the City of Newark wishes to agree to any amendments to the Grant(s).
 
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
    1. The Mayor and/or his designee, the Director of the Department of Child and Family Well-Being are hereby authorized to submit an application to the Department of Health and Human Services/Health Resources and Services Administration (HRSA), Hubert H. Humphrey Building, 200 Independence Avenue, S.W., Washington, DC 20201, requesting grant funding in the amounts of $1,606,073.00 and $35,000.00 for the continued provision of health care, social services, substance abuse and mental health services, as well as enhanced access to medical care and increased hours of operation of services to Newark's homeless population with no City funds required (a copy of the application is attached hereto and made a part hereof).
    2. The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.
    1. The grant awards in the amount of $669,177.00 and $35,000.00 for a total amount of $704,177.00 from the Department of Health and Human Services/Health Resources and Services Administration (HRSA) is hereby accepted (a copy of the award letter is attached hereto and made a part hereof).
    1. The grant agreements between the City of Newark and Department of Health and Human Services/Health Resources and Services Administration (HRSA) are hereby ratified from November 1, 2011 to the date of adoption of this authorizing resolution.
    2. No municipal funds are required for receipt or acceptance of this grant award.
    3. The Director of the Department of Child and Family Well-Being shall file fully executed copies of the resolution, grant applications and grant award letters with the City of Newark's City Clerk.  
 
STATEMENT
 
This resolution ratifies and authorizes the actions taken by the Mayor and the Director of Child and Family Well-Being to apply for and accept grant funds in the amount of $669,177.00 and $35,000.00, for a total amount of $704,177.00 from the Department of Health and Human Services/Health Resources and Services Administration (HRSA), which funds shall be used to provide medical care, social services, substance abuse and mental health services, as well as enhance access to medical care and increase hours of operation of services to Newark's homeless population, for the period commencing on November 1, 2011 and terminating on October 31, 2012. No municipal funds are required from the municipal budget.
