WHEREAS, on or about March 20, 2020, Integrity, Inc., proposed a collaboration with the City of Newark, Department of Health and Community Wellness (the “City”) to provide primary medical care services at Integrity’s Certified Community Behavioral Health Center, 1091 Broad Street, Newark, New Jersey 07102; and
WHEREAS, Integrity, Inc., 103 Lincoln Park, Newark, New Jersey 07102 made funds available to provide medical care services; and
WHEREAS, Integrity, Inc., will fund and provide the City with primary care services in the amount of Two Hundred Thirty-Three Thousand One Hundred Twenty Dollars and Zero Cents ($233,120.00), to support the medical staffing for the period of October 1, 2020 through September 30, 2021 with an a option to renew for an additional year for the same amount (a copy of the Agreement is attached hereto and made a part hereof); and
WHEREAS, Integrity Inc., may amend the grant to reflect a grant time extension and/or increase or decrease of additional funds if made available; and
WHEREAS, the City shall, upon any such extension of time of the grant and/or increase or decrease of additional funds, cause a budget insertion to be submitted to the Municipal Council for its consideration in the amount of the grant.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to accept the funds from Integrity Inc. to support the City’s medical staffing needs at 1091 Broad Street, Newark, New Jersey 07102, in the amount of Two Hundred Thirty-Three Thousand One Hundred Twenty Dollars and Zero Cents ($233,120.00), with the option to increase the grant award amount up to an additional $233,120.00.
2. The resulting funding award in the amount of Two Hundred Thirty-Three Thousand, One Hundred Twenty Dollars and Zero Cents ($233,120.00) from Integrity, Inc., is hereby accepted.
3. The Municipal Council hereby ratifies the acceptance of this grant award from October 1, 2020 to the date of adoption of this authorizing resolution.
4. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to receive and expend the grant award to provide medical services at 1091 Broad Street, Newark, New Jersey 07102, for the period commencing October 1, 2020 and ending September 20, 2021.
5. The City’s Department of Health and Community Wellness shall, upon any extension of time of the grant, and/or increase or decrease of additional funds, cause a budget insertion to be submitted to the Municipal Council for its consideration.
6. No municipal matching funds are required for the receipt or acceptance of this grant award.
STATEMENT
This resolution ratifies and authorizes the actions taken by the Mayor and/or his designee, the Director of Health and Community Wellness, to accept grant funds in the amount of $233,120.00, from Integrity, Inc., to provide medical services at 1091 Broad Street, Newark, New Jersey 07102, for the period commencing October 1, 2020 and ending September 30, 2021, with an option to increase the grant award amount up to an additional $233,120.00, if the parties agree to continue the relationship for an additional year. No municipal matching funds are required for the receipt or acceptance of this grant award.
