WHEREAS, Donna Sapienza (Ms. Sapienza) has been an employee of the City of Newark, (the “City”) as a police officer from March 16, 1998 through the present; and
WHEREAS, Ms. Sapienza is an active member of the Police and Firemen’s Retirement System (PFRS) and has twenty-two (22) years, ten (10) months of PFRS Service Credit, meeting the minimum service requirement of at least four (4) years of PFRS membership; and
WHEREAS, the Acting Director of the Department of Public Safety, based upon the advice and documentation provided by physicians, certifies that Ms. Sapienza is unable to perform her assigned duties; and
WHEREAS, the City is unable to provide an alternative to the PFRS covered position with duties capable of being performed by Ms. Sapienza; and
WHEREAS, the Acting Director of the Department of Public Safety has consented to the processing of the Involuntary Disability Retirement application.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Business Administrator, the Acting Director of the Department of Public Safety and the Director of Personnel are authorized to process the Involuntary Disability Retirement application for Donna Sapienza.
2. Certified copies of this Resolution shall be forwarded to all parties involved in the administration of this action.
