Additional Information: 

On October 5, 2016, pursuant to Resolution 7R2-c, the DHCW accepted $12,000.00 from the DoAS. On October 31, 2017, the DoAS notified the City of Newark that it was awarding it no-cost extension dollars in the amount of $3,000.00, thus extending the contract period and bringing the total Program grant amount to $15,000.00.

WHEREAS, the New Jersey Department of Human Services, Division of Aging Services (DoAS), announced the availability of Falls Prevention Programs grant funds to increase the number of New Jersey’s seniors and adults with disabilities at risk of falling who successfully complete Falls Prevention Programs; and

 

WHEREAS, the City of Newark desires to satisfy the public interest in protecting New Jersey’s seniors and disabled by obtaining funding from the DoAS Falls Prevention grant program; and

 

WHEREAS, on or about July 20, 2016, the Director of the Department of Health and Community Wellness (DHCW), on behalf of the City of Newark (hereinafter “City”), submitted an application (hereinafter “Application”) to the New Jersey Department of Human Services, Division of Aging Services (DoAS), whose address is P.O. Box 807, Trenton, New Jersey 08625. The Application was a request for Falls Prevention Programs funds in the amount of $12,000.00 (hereinafter “Grant”), for a project period from August 1, 2016 through July 31, 2017 (a copy of said application is attached hereto and made a part hereof); and

 

WHEREAS, pursuant to Resolution 7R2-c (attached), adopted on October 5, 2016, the DHCW applied for and accepted $12,000.00 from the DoAS, for the period of August 1, 2016 through July 31, 2017, for the purpose of increasing the number of New Jersey’s seniors and adults with disabilities at risk of falling who successfully complete A Matter of Balance: Managing Concerns about Falls (MOB) -- a falls prevention program introduced by the DoAS; and   

 

WHEREAS, on October 31, 2017, as an additional result of the City’s Application, the DoAS increased the Grant by awarding the City additional funds in the amount of Three Thousand Dollars and Zero Cents ($3,000.00), thereby extending the Grant period from August 1, 2017 through March 31, 2018. The purpose of the additional funding is for the City to continue offering MOB workshops for seniors and adults with disabilities; and 

 

WHEREAS, the DHCW, desires to accept the additional funds in the amount of Three Thousand Dollars and Zero Cents ($3,000.00); and   

 

           WHEREAS, all previous terms and conditions of the Grant remain the same.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1,                     Pursuant to Resolution 7R2-c (attached), adopted October 5, 2016, the Newark Municipal Council authorized the Mayor and/or his designee, the Director of the Department of Health and Community Wellness, to submit an application to New Jersey Department of Human Services, Division of Aging Services (DoAS), whose address is P.O. Box 807, Trenton, New Jersey 08625-0807, requesting Falls Prevention Programs grant funds in the amount of Twelve Thousand Dollars and Zero Cents ($12,000.00) with no City funds required (a copy of the application is attached hereto and made a part hereof). The purpose of those grant funds was to increase the number of New Jersey’s seniors and adults with disabilities at risk of falling who successfully complete A Matter of Balance: Managing Concerns about Falls (MOB) workshops during the grant period of August 1, 2016 through July 31, 2017. 

 

2.                     The additional grant funds in the amount of Three Thousand Dollars and Zero Cents ($3,000.00), from the DoAS are hereby accepted; a copy of the DoAS Notice of Intent is attached hereto and made a part hereof. 

 

3.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, hereby accept these additional grant funds in the amount of Three Thousand Dollars and Zero Cents ($3,000.00), which will bring the total MOB grant amount to $15,000.00 and will extend the contract period from August 1, 2017 through March 31, 2018. 

 

4.                     The Municipal Council hereby ratifies the acceptance of these additional grant funds from August 1, 2017, to the date of adoption of this authorizing resolution.

 

5.                     No municipal funds are required for receipt or acceptance of this grant award.

 

6.                     The Director of the Department of Health and Community Wellness shall file a fully-executed copy of the DoAS Notice of Intent in the Office of the City Clerk.  

 

STATEMENT

 

This resolution authorizes the Mayor and/or his designee, the Director of the Department of Health and Community Wellness, to accept additional funds in the amount of Three Thousand Dollars and Zero Cents ($3,000.00), from the New Jersey Department of Human Services, Division of Aging Services (DoAS), P.O. Box 807, Trenton, New Jersey 08625-0807, and extend the grant period from August 1, 2017 through March 31, 2018. No municipal funds are required from the City for the acceptance of the grant funds.
