WHEREAS, the City of Newark desires to improve on clinical  outcomes under the Heart Disease and Stroke Program Proposal by modifying, upgrading and the purchase of Practice Management/Electronic Medical Records (PM/EMR) software modules, which would allow for capturing data, establishing base lines and assessing health outcomes for residents of the City of Newark; and

 

WHEREAS, the New Jersey Department of Health and Senior Services has made grant funds available that can be used to modify, upgrade and purchase PM/EMR software modules; and

 

WHEREAS, on June 30, 2016, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness submitted an application to the New Jersey Department of Health and Senior Services, P.O. Box 360, Trenton, New Jersey 08625, requesting grant funding in the amount of seventy-six thousand, four hundred sixty-seven dollars and zero cents ($76,467.00), to improve on clinical outcomes under the Heart and Stroke Program Proposal, for modifying, upgrading and the purchase of PM/EMR software modules, which would allow for capturing data, establishing base lines and assessing health outcomes for residents of the City of Newark (a copy of the Grant Application is attached hereto and made a part hereof); and

 

WHEREAS, as a result of this application, on August 4, 2016, the New Jersey Department of Health and Senior Services issued the City of Newark a Letter of Intent in the amount of seventy-six thousand, four hundred sixty-seven dollars and zero cents ($76,467.00), to improve on clinical outcomes under the Heart and Stroke Program Proposal,  for modifying, upgrading and the purchase of PM/EMR software modules, which would allow for capturing data, establishing base lines and assessing health outcomes for residents of the City of Newark, for the period commencing on October 1, 2016, and terminating on June 30, 2017 (a copy of the Letter of Intent is attached hereto and made a part hereof); and

 

WHEREAS, the City of Newark’s Department of Health and Community Wellness desires to accept said grant funds in the amount of seventy-six thousand, four hundred sixty-seven dollars and zero cents ($76,467.00); and

 

WHEREAS, the City of Newark recognizes that the New Jersey Department of Health and Senior Services may extend the time of the grant award and/or provide additional funds if made available; and

 

WHEREAS, in such cases, the New Jersey Department of Health and Senior Services will amend the grant to reflect the grant time extension and/or increase or decrease of grant funds available; and

 

WHEREAS, in such cases, the City of Newark’s Department of Health and Community Wellness shall, upon any extension of time of the grant and/or increase or decrease of grant funds, file with the City of Newark’s Municipal Council for their consideration a proposed amending resolution reflecting the final term and award amount of the grant; and 

 

WHEREAS, terms of this grant award may only be waived or amended by the parties duly authorized to execute the grant, subject to the approval of the Newark Municipal Council by resolution.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to submit an application to the New Jersey Department of Health and Senior Services, P.O. Box 360, Trenton, New Jersey 08625, requesting grant funds in the amount of seventy-six thousand, four hundred sixty-seven dollars and zero cents ($76,467.00), to improve on clinical outcomes under the Heart and Stroke Program Proposal, for modifying, upgrading and the purchase of PM/EMR software modules, which would allow for capturing data, establishing base lines and assessing health outcomes for residents of the City of Newark, for the period commencing on October 1, 2016, and terminating on June 30, 2017.

 

2.                     The Municipal Council hereby ratifies its authorization of this application from the date of the application’s submission to the date of adoption of this authorizing resolution.

 

3.                     The grant award in the amount of seventy-six thousand, four hundred sixty-seven dollars and zero cents ($76,467.00) by the New Jersey Department of Health and Senior Services, is hereby accepted.

 

4.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to receive and expend the grant award to improve on clinical outcomes under the Heart and Stroke Program Proposal, by modifying, upgrading and the purchase of PM/EMR software modules, for the period commencing on October 1, 2016, and terminating on June 30, 2017.

 

5.                     The City of Newark’s Department of Health and Community Wellness shall, upon any extension of time of the grant and/or increase or decrease of grant funds, file with the City of Newark’s Municipal Council for their consideration a proposed amending resolution reflecting the final length of time and award amount of the grant. 

 

6.                     The Municipal Council hereby ratifies the acceptance of these grant funds from October 1, 2016, to the date of adoption of this authorizing resolution.

 

7.                     No municipal funds are required for receipt or acceptance of this grant award.            

 

STATEMENT

 

This resolution ratifies and authorizes the actions taken by the Mayor and/or his designee, the Director of the Department of Health and Community Wellness, to apply for and accept grant funds in the amount of Seventy-Six Thousand, Four Hundred Sixty-Seven Dollars and Zero Cents ($76,467.00), from the New Jersey Department of Health and Senior Services, to improve on clinical outcomes under the Heart and Stroke Program Proposal, by modifying, upgrading and the purchase of PM/EMR software modules, which will allow for capturing data, establishing base lines and assessing health outcomes for residents of the City of Newark, for the period commencing on October 1, 2016, and terminating on June 30, 2017.  No municipal funds are required for the acceptance of the grant funds.
