WHEREAS, on or about June 5, 2023, Integrity, Inc., proposed a collaboration with the City of Newark, Department of Health and Community Wellness (the “City”) and the Mary Eliza Mahoney FQHC, to provide primary health care services at Integrity’s Certified Community Behavioral Health Center, located at 1091 Broad Street, Newark, New Jersey 07102; and

 

WHEREAS, Integrity, Inc., 103 Lincoln Park, Newark, New Jersey 07102 made funds available to provide primary health care services; and

 

                     WHEREAS, Integrity, Inc., will fund and provide the City with primary health care services in the amount of Sixty Thousand Dollars and Zero Cents ($60,000.00), to support the medical staffing needs for the period of February 1, 2023 through January 31, 2024, with an a option to renew for an additional year, for an additional $60,000.00 (a copy of the Business Associate Agreement [BAA] is attached hereto and made a part hereof); and

 

WHEREAS, the Director of the Department of Health and Community Wellness desires to enter into and execute the BAA with Integrity, Inc., to accept funds in the amount Sixty Thousand Dollars and Zero Cents ($60,000.00), for the provision of medical staffing needs for the period of February 1, 2023 through January 31, 2024 with an a option to renew for an additional year for the same amount; and

 

WHEREAS, Integrity Inc., may amend the grant to reflect a grant time extension and/or increase or decrease of additional funds if made available; and

 

WHEREAS, the City shall, upon any such extension of time of the grant and/or increase or decrease of additional funds, cause a budget insertion to be submitted to the Municipal Council for its consideration in the amount of the  grant.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to enter into and execute a Business Associate Agreement (attached hereto) to accept the funds from Integrity, Inc. to support the City’s medical staffing needs, in the amount of Sixty Thousand Dollars and Zero Cents ($60,000.00), with the option to increase the grant award amount up to an additional $60,000.00.

 

2.                     The resulting funding award in the amount of Sixty Thousand Dollars and Zero Cents ($60,000.00) from Integrity, Inc., is hereby accepted.

 

3.                     The Municipal Council hereby ratifies the acceptance of this grant award from February 1, 2023 to the date of adoption of this authorizing resolution.

 

4.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to receive and expend the grant award to provide medical services at 1091 Broad Street, Newark, New Jersey 07102, for the period commencing February 1, 2023 and ending January 31, 2024.

 

5.                     The City’s Department of Health and Community Wellness shall, upon any extension of time of the grant, and/or increase or decrease of additional funds, cause a  budget insertion to be submitted to the Municipal Council for its consideration.

 

6.                     No municipal matching funds are required for the receipt or acceptance of this grant award.

 

7. This award is hereby ratified from February 1, 2023 through the date of adoption of this authorizing resolution.

 

 

STATEMENT

 

This resolution ratifies and authorizes the actions taken by the Mayor and/or his designee, the Director of Health and Community Wellness, to enter into and execute a Business Associate Agreement to accept grant funds in the amount of Sixty -Thousand Dollars and Zero Cents ($60,000.00), from Integrity, Inc., to support medical staffing needs, for the period commencing February 1, 2023 and ending January 31, 2024, with an option to increase the grant award amount up to an additional $60,000.00, if the parties agree to continue the relationship for an additional year.  No municipal matching funds are required for the receipt or acceptance of this grant award.
