Goal: The Department of Health and Community Wellness will offer a variety of immunization services to community residents' ages 0 through 18 years.

WHEREAS, the Department of Health and Community Wellness provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and
WHEREAS, the State of New Jersey Department of Health - Vaccine Preventable Disease Program has made funds available for the provision of such services; and
WHEREAS, on or about April 28, 2025, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness (the "City"), submitted an application to the State of New Jersey Department of Health, Division of Epidemiology, Environmental and Occupational Health, P.O. Box 369, Trenton, New Jersey 08625 ("State of New Jersey, Department of Health"), requesting grant funding in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), for the continued provision of Immunization Project Vaccinate Program for immunization service delivery, vaccine storage and handling, community outreach and case management for children (a copy of the application is attached here to and made a part hereof); and
WHEREAS, as a result of this Application ("Application"), the City of Newark has been awarded a grant in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00) ("Grant Funds"), from the State of New Jersey, Department of Health, for the continued provision of Immunization Project Vaccinate Program for immunization service delivery, vaccine storage and handling and community outreach for children and adults, for the period commencing on July 1, 2025 and terminating on June 30, 2026 (a copy of the grant award letter is attached hereto and made a part hereof)("Grant Award"); and
WHEREAS, the City's Department of Health and Community Wellness, desires to accept the Grant Funds; and
WHEREAS, the City of Newark recognizes that the State of New Jersey, Department of Health, may extend the time of the Grant Award and/or provide additional funds, if made available; and
WHEREAS, the City's Department of Health and Community Wellness shall, upon any extension of time of the grant and or increase or decrease of additional funds, file an amending fiscal resolution reflecting the extension of time and award amount of the grant.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Mayor of the City of Newark and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to submit an Application to the State of New Jersey, Department of Health, Division of Epidemiology, Environmental and Occupational Health ("State of New Jersey, Department of Health"), P.O. Box 369, Trenton, New Jersey 08625, requesting grant funding in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), for the continued provision of Immunization Action Plan/Project Vaccinate Program for immunization service delivery, vaccine storage and handling, and community outreach for children ("Application").
2. The Municipal Council hereby ratifies its authorization of this Application from the date of Application submission to the date of adoption of this authorizing Resolution.
3. The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to accept the grant award in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00) ("Grant Funds"), from the State of New Jersey, Department of Health.
4. The Mayor of the City of Newark and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to receive and expend the Grant Funds in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), for the continued provision of immunization service delivery, vaccine storage and handling, and community outreach for children, for the period commencing on July 1, 2025 and terminating on June 30, 2026.
5. The Agreement between the City and the State of New Jersey, Department of Health is hereby ratified from date of execution to the date of adoption of this authorizing Resolution.
6. No municipal funds are required for receipt or acceptance of the Grant Funds.
7. The Director of the Department of Health and Community Wellness shall file a fully executed copy of the Application and Grant Award letter in the Office of the City Clerk.
STATEMENT
This Resolution ratifies and authorizes the actions taken by the Mayor of the City of Newark and or/his designee, the Director of the Department of Health and Community Wellness, to apply for and accept a grant award in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), from the State of New Jersey, Department of Health, Division of Epidemiology, Environmental, and Occupational Health, P.O. Box 369, Trenton, New Jersey 08625, for the continued provision of Immunization/Project Vaccinate Program for immunization service delivery, vaccine storage and handling, and community outreach for the period commencing on July 1, 2025 and terminating on June 30, 2026. No municipal funds are required for receipt or acceptance of the grant award.
