WHEREAS, pursuant to Resolution 7RI-G, authorized on December 21, 2010, the Municipal Council authorized the Mayor and/or his designee, the Deputy Mayor/Director of the Department of Economic and Housing Development to enter into and execute a contract with the Jewish Renaissance Medical Center, (hereinafter referred to as the Sub-recipient) to provide physical rehabilitation services to its facility located at 359 13th Avenue, Newark, NJ 07103 for the period of January 1, 2010 to December 31, 2010, in an amount not to exceed $30,000.00; and
WHEREAS, pursuant to Resolution 7R3-C, authorized on June 02, 2011, the Municipal Council authorized the Mayor and/or his designee, the Deputy Mayor/Director of the Department of Economic and Housing Development to enter into and execute a first amended contract with the Sub-recipient for the purpose of extending the contract period from January 1, 2010 - December 31, 2010 to January 1, 2010 - December 31, 2011, to allow the Sub-recipient to complete the physical rehabilitation to its facility located at 359 13th Avenue, Newark, NJ, 13th Avenue School Health Center Alliance, Block 1798, Lot 9/West Ward; and
WHEREAS, the Sub-recipient has requested authorization for a second amended contract extending the contract period for an additional nine months for an amended period of January 1, 2010-September 30, 2012, to allow the Sub- recipient to complete the physical rehabilitation to its facility located at 359 13th Avenue, Newark, NJ, 13th Avenue School Health Center Alliance, Block 1798, Lot 9/West Ward; and
WHEREAS, this project does not require the use of additional funds and all other terms and conditions contained in contract authorized under Resolution 7R3-C060211 shall remain in full effect.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Mayor and/or his designee, the Deputy Mayor/Director of the Department of Economic and Housing Development, are hereby authorized to enter into and execute a second amended contract, in the form attached hereto, with the Jewish Renaissance Medical Center, 275 Hobart Street, Perth Amboy, NJ 08861, (hereinafter referred to as the Sub-recipient) to amend the term of the contract from January 1, 2010-December 31, 2011 to January 1, 2010-September 30, 2012, to allow the Sub-recipient to complete the physical rehabilitation to its facility located at 359 13th Avenue, Newark, NJ, 13th Avenue School Health Center Alliance, Block 1798, Lot 9/West Ward.
2. The second amended contract between the City of Newark and the sub-recipient is hereby ratified from December 31, 2011 to the date of adoption of this authorizing resolution.
3. This resolution and a fully executed copy of the second amended contract shall be filed with the Office of the City Clerk by the Deputy Mayor/Director of the Department of Economic and Housing Development.
4. This project does not require the use of additional funds and all other terms and conditions contained in the contract authorized pursuant to Resolution 7R3-c 060211 shall remain in full effect and in accordance with the law.
