WHEREAS, the Department of Health and Community Wellness provides immunization services, which include but are not limited to, immunization service delivery; vaccine storage and handling; and community outreach and case management, in an effort to increase immunization coverage levels for children and adults, known as the Immunization Action Plan/Project Vaccinate Program in the City of Newark ("City"); and

WHEREAS, the State of New Jersey Department of Health - Communicable Disease Service ("NJDOH") has made funds available for the provision of such services; and

WHEREAS, on April 4, 2024, the NJDOH sent the City a Letter of Intent to Fund ("LOI") informing of the intent to renew the current grant award #EPID24OMM007 in the amount of Three Hundred Eighty Eight Thousand Dollars and Zero Cents ($388,000.00) for FY25, subject to the availability of funds, for the grant period of July 1, 2024 through June 30, 2025 (a copy of SFY25 Letter of Intent to Fund - Immunization 2025 is attached hereto and made a part hereof); and
WHEREAS, upon receipt of the LOI, the City was required to submit an application, and as a result of this application, the City shall be awarded a grant in the amount of $388,000.00 from the NJDOH; and

WHEREAS, the City's Department of Health and Community Wellness desires to accept the funds in the amount of $388,000.00; and

WHEREAS, the City recognizes that the NJDOH may extend the time of the grant award and/or provide additional funds, if made available; and

WHEREAS, the City's Department of Health and Community Wellness shall, upon any extension of time of the grant and or increase or decrease of additional funds, file an amending fiscal resolution reflecting the extension of time and award amount of the grant.

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

1. The Mayor of the City of Newark ("City"), and/or his designee, the Director of the Department of Health and Community Wellness, is hereby authorized to submit an application to the State of New Jersey, Department of Health - Communicable Disease Service ("NJDOH") , P.O. Box 369, Trenton, New Jersey 08625, requesting grant funding in the amount of Three Hundred Eighty-Eight Thousand Dollars and Zero Cents ($388,000.00), for immunization service delivery; vaccine storage and handling; and community outreach and case management, in an effort to increase immunization coverage levels for children and adults known as the Immunization Action Plan/Project Vaccinate Program.

2. The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing Resolution.

3. The grant award in the amount of $388,000.00 from the NJDOH is hereby accepted.

4. The Mayor of the City, and/or his designee, the Director of the Department of Health and Community Wellness, is hereby authorized to enter into and execute grant documents with the NJDOH for the Immunization Action Plan/Project Vaccinate Program, for the period commencing on July 1, 2024 through June 30, 2025.

5. The Municipal Council hereby ratifies the acceptance of this grant award to the date of adoption of this authorizing Resolution.

6. The City's Department of Health and Community Wellness shall, upon any extension of time of the grant and/or increase or decrease of additional funds, file an amending resolution reflecting the extension time and award amount of the grant.

7. No municipal funds are required for receipt or acceptance of this grant award.

8. The Director of the Department of Health and Community Wellness shall file a fully executed copy of the grant application and grant award letter in the Office of the City Clerk.


STATEMENT

This Resolution ratifies and authorizes the actions by the Mayor of the City of Newark and or/his designee, the Director of the Department of Health and Community Wellness, to apply for and accept a grant award in the amount of Three Hundred Eighty Eight Thousand Dollars and Zero Cents ($388,000.00), from the State of New Jersey, Department of Health - Communicable Disease Service, P.O. Box 369, Trenton, New Jersey 08625, for the immunization service delivery, vaccine storage and handling, community outreach and case management in an effort to increase immunization coverage levels for children and adults, known as the Immunization Action Plan/Project Vaccinate Program, for the period commencing on July 1, 2024 through June 30, 2025. No municipal funds are required for receipt or acceptance of the grant award.
