WHEREAS, the Department of Health and Community Wellness provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and

WHEREAS, the State of New Jersey Department of Health - Vaccine Preventable Disease Program has made funds available for the provision of such services; and

WHEREAS, on May 5, 2022, on behalf of the City of Newark, the Acting Director of the Department of Health and Community Wellness (the "City"), submitted an application to the State of New Jersey Department of Health - Vaccine Preventable Disease Program (Immunization 2023), P.O.
