WHEREAS, the City of Newark provides community outreach, education and medical services related to sexually transmitted diseases, including but not limited to, high quality medical and laboratory services to residents of the City of Newark at the City's Communicable Disease Prevention and Treatment Center (CDPTC); and

WHEREAS, the New Jersey Department of Health and Senior Services (hereinafter "NJDOH") has made grant funds available for the provision of such services at the CDPTC; and

WHEREAS, on May 16, 2017, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness (hereinafter "DHCW") submitted an application to the NJDOH, P.O. Box 360, Trenton, New Jersey 08625, requesting grant funding in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), for the continued provision of community outreach, education and medical services related to sexually transmitted diseases for the period commencing on July 1, 201...
