WHEREAS, the City of Newark's (hereinafter, "City") Department of Health and Community Wellness (hereinafter, "DHCW") provides medical care services pursuant to and consistent with the Health Center Program laws and regulations (including Section 330 of Public Health Services Act, 42 U.S.C. Section 254b and 42 C.F.R. ? 51c.301, et seq.); and
WHEREAS, the State of New Jersey Department of Health-Division of Family Health Services, P.O. Box 364, Trenton, New Jersey 08625- 0355 ("NJDOH"), has made grant funds available to improve the health outcomes of women served in the city of Newark and surrounding areas; and
WHEREAS, on May 8, 2026, on behalf of the City of Newark, the Director of the DHCW submitted an application to the State of New Jersey Department of Health, Division of Family Health Services, requesting grant funding in the amount of One Hundred Nine Thousand, Three Hundred Dollars and Zero Cents ($109,300.00) (a copy of the Grant Application is attached hereto and made a part hereof); and
