WHEREAS, Delta Dental of New Jersey Foundation, Inc. ("Delta Dental") has made grant funds available to provide oral health care to New Jersey's uninsured residents and to improve oral health care challenges found in the Community; and
WHEREAS, on or about February 16, 2024, on behalf of the City of Newark ("City"), the Director of the Department of Health and Community Wellness ("DHCW") submitted a grant application to Delta Dental (a copy of the grant application is attached hereto and made a part hereof); and
WHEREAS, as a result of the grant application, on or about April 18, 2024, Delta Dental awarded the City a grant in the amount of Ten Thousand Dollars and Zero Cents ($10,000.00), for the period of July 1, 2024 through June 30, 2025 (a copy of the executed Contract is attached hereto and made a part hereof); and
WHEREAS, the Director of the DHCW desires to accept said grant funds; and
WHEREAS, the City recognizes that Delta Dental may extend the time of the grant award and/or provide additional funds when it becomes available; and
