WHEREAS, Greater Newark Health Care Coalition Inc. ("GNHCC"), 80 Main Street, Suite 210, West Orange NJ 07052, has been awarded the multi-year Grant from New Jersey State Department of Health; and
WHEREAS, the New Jersey State Department of Health approved released carryover funds leading to an award to the City of Newark (the "City"); and
WHEREAS, GNHCC has allocated Grant Funds to the City, for the implementation of Hypertensive quality project for the Newark Learning Collaborative in the amount of One Hundred Thousand Dollars and Zero Cents ($100,000.00), for the Grant Period of July 1, 2025 through September 29, 2026; and
WHEREAS, the Director of the Department of Health and Community Wellness ("DHCW") desires to accept said grant funds; and
WHEREAS, the City recognizes that GNHCC may extend the period of the grant award and/or provide additional funds when it becomes available; and
