WHEREAS, the City of Newark's Department of Health and Community Wellness (hereinafter "DHCW") provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark (hereinafter "City"); and
WHEREAS, the State of New Jersey Department of Health (NJDOH)-Vaccine Preventable Disease Program (hereinafter "State Vaccine Program") has made funds available for the provision of such services; and
WHEREAS, on December 18, 2016, on behalf of the City, the Acting Director of DHCW submitted an application to the State Vaccine Program, P.O. Box 369, Trenton, New Jersey 08625, requesting grant funding in the amount of five hundred and twenty-four thousand eight hundred sixty-seven dollars and zero cents ($524,867.00), for the continued provision of the City's Immunization Project Vaccinate Program for immunization service delivery, vaccine storage and handling, community outreach and case management for children (a copy of the applicat...
