WHEREAS, the City of Newark, Department of Health and Community Wellness, desires to provide medical care services pursuant to and consistent with the National Hypertension Control Initiative to address hypertension (high blood pressure) among the City of Newark racial and ethnic minority populations, and
WHEREAS, the United States Department of Health and Human Resources Health, Health Resources and Services Administration (HRSA), has made grant funds available to address hypertension (high blood pressure) among racial and ethnic minority populations, and
WHEREAS, on December 4, 2020, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness (the "City") electronically filed an application to HRSA, requesting grant funding in the amount of One Hundred Thirty-Four Thousand Four Hundred Dollars and Zero Cents ($134,400.00), for the Hypertension Control Initiative (a copy of the application is attached hereto and made a part hereof); and
