WHEREAS, the City of Newark's (hereinafter, "City") Department of Health and Community Wellness (hereinafter, "DHCW") provides medical care services pursuant to and consistent with the Health Center Program laws and regulations (including Section 330 of Public Health Services Act, 42 U.S.C. Section 254b and 42 C.F.R. ? 51c.301, et seq.); and
WHEREAS, Acenda, Inc. ("Acenda") has made grant funds available to expand services through the use of Community Health Workers ("CHW") by integrating CHWs into health and care teams to assist in various needs found in the Community; and
WHEREAS, on or about September 22, 2023, on behalf of the City, the Director of the DHCW submitted a grant application to Acenda (a copy of the grant application is attached hereto and made a part hereof); and
WHEREAS, as a result of the grant application, on or about October 11, 2023, Acenda awarded the City a grant in the amount of Sixty Six Thousand Six Hundred Sixty-Seven Dollars and Zero Cents ($66,667.00), for the period of September 1, 2023 through June 30, 2024 (a copy of the execut...
