WHEREAS, the Petitioner, Justine Branham, instituted suit in the Division of Workers Compensation of New Jersey, Essex County, bearing C.P. Number 2014-28634 against the City of Newark seeking workers' compensation benefits as a result of an accident that she sustained on December 10, 2013, when she slipped and fell on icy stairs at the rear exit of the 4th Precinct, while working as a Police Officer for the Newark Department of Public Safety, Police Division; and
WHEREAS, following settlement negotiations through Petitioner's attorney, Markman & Cannan, L.L.C., and Corporation Counsel and/or his designee, Chasan Leyner & Lamparello, P.C. ("Outside Counsel") have agreed to amicably resolve and settle this matter under Section 22 of the Workers Compensation Act (N.J.S.A. 34:15-22) by payment in the amount of Forty Two Thousand Nine Hundred Six Dollars and Zero Cents ($42,906.00). Pursuant to the terms of the Settlement Order to be executed by the Judge of the Division of Workers' Compensation, the City of Newark will also pay an attorney fee of Five Thousand One Hundred Forty Nine Dollars and Zero Cents ($5,149.00), a medical evaluation fee of Four Hundred Dollars and Zero Cents ($400.00) and a transcription fee of Ninety Dollars and Zero Cents ($90.00) for a total settlement of Forty Eight Thousand Five Hundred Forty Five Dollars and Zero Cents ($48,545.00); and
