WHEREAS, on or about March 20, 2020, Integrity, Inc., proposed a collaboration with the City of Newark, Department of Health and Community Wellness (the "City") to provide primary medical care services at Integrity's Certified Community Behavioral Health Center, 1091 Broad Street, Newark, New Jersey 07102; and
WHEREAS, Integrity, Inc., 103 Lincoln Park, Newark, New Jersey 07102 made funds available to provide medical care services; and
WHEREAS, Integrity, Inc., will fund and provide the City with primary care services in the amount of Two Hundred Thirty-Three Thousand One Hundred Twenty Dollars and Zero Cents ($233,120.00), to support the medical staffing for the period of October 1, 2020 through September 30, 2021 with an a option to renew for an additional year for the same amount (a copy of the Agreement is attached hereto and made a part hereof); and
