WHEREAS, the Department of Health and Community Wellness provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and
WHEREAS, the New Jersey Department of Health (NJDOH), Vaccine Preventable Disease Program, has made additional funds available for the provision of such services through the Immunization 2021 Grant; and
WHEREAS, on June 2, 2021, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness (the "City"), submitted an electronic application to the NJDOH, Vaccine Preventable Disease Program, for the Immunization 2021 Grant, requesting grant funding in the amount of Five Hundred Fifty-Five Thousand, Five Hundred and Eighty-Six Dollars and Zero Cents ($555,586.00), to continue support to selected initiatives to increase immunization levels; and
