WHEREAS, on or about June 5, 2023, Integrity, Inc., proposed a collaboration with the City of Newark, Department of Health and Community Wellness (the "City") and the Mary Eliza Mahoney FQHC, to provide primary health care services at Integrity's Certified Community Behavioral Health Center, located at 1091 Broad Street, Newark, New Jersey 07102; and
WHEREAS, Integrity, Inc., 103 Lincoln Park, Newark, New Jersey 07102 made funds available to provide primary health care services; and
WHEREAS, Integrity, Inc., will fund and provide the City with primary health care services in the amount of Sixty Thousand Dollars and Zero Cents ($60,000.00), to support the medical staffing needs for the period of February 1, 2023 through January 31, 2024, with an a option to renew for an additional year, for an additional $60,000.00 (a copy of the Business Associate Agreement [BAA] is attached hereto and made a part hereof); and
