WHEREAS, the City of Newark's, Department of Health and Community Wellness ("DHCW") is a Local Health Department and is co-affiliated with a Federally Qualified Health Center, Mary Eliza Mahoney Health center (hereinafter, the "City"; and
WHEREAS, RWJBH Corporate Services, Inc., 95 Old Short Hills Road, West Orange, New Jersey 07052 (hereinafter "RWJBH"), is a non-profit health organization organized under the laws of the State of New Jersey and which operates a licensed acute care facility providing inpatient, outpatient and emergency services in Newark; and
WHEREAS, RWJBH and City have identified that there is a need for expanded access to comprehensive high-quality outpatient services, including specialty services, for the residents of their shared service area, particularly among City's target population of low income and medically underserved individua...
