WHEREAS, the City of Newark provides sexually transmitted diseases outreach, educational and medical services, including, but not limited to, high quality medical and laboratory services, to residents of the City of Newark at the City's Communicable Disease Prevention and Treatment Center (CDPTC); and

WHEREAS, the New Jersey Department of Health and Senior Services has made grant funds available for the provision of such services at the CDPTC; and

WHEREAS, on April 20, 2016, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness submitted an application to the New Jersey Department of Health and Senior Services, P.O. Box 360, Trenton, New Jersey 08625, requesting grant funding in the amount of one hundred twenty-four thousand, one hundred thirteen dollars and zero cents ($124,113.00), for the continued provision of sexually transmitted diseases outreach, educational and medical services for the period commencing on July 1, 2016, and terminating on June 30, 2017 (a copy of the Gr...
