Whereas, the Insurance Fund Commission of the City of Newark is in need of a Third Party Administrator to assist the City of Newark with the administration of its Self-Insured Workers’ Compensation Program; and
WHEREAS, by Resolution 7R7-f, adopted March 16, 2011, the Municipal Council authorized the Mayor and Business Administrator to enter into a contract with Cannon Cochran Management Services, Inc., (CCMSI), 3535 Route 66, Building 6, Neptune, NJ 07752, for the period of December 5, 2010 and terminating on December 4, 2011 for administration consulting and management services as the Third-Party Administrator of the Self-Insured Workers’ Compensation program (hereinafter, the “Contract”:); and
WHEREAS, the Contract was for a term of one year, commencing on December 5, 2010 and terminating on December 4, 2011, with a one (1) year extension provision, subject to the approval of the Municipal Council; and
