WHEREAS, the City of Newark provides sexually transmitted diseases outreach and educational services to include, but not limited to, high quality medical and laboratory services to residents in the City of Newark, requiring services at the Communicable Disease Prevention and Treatment Center (CDPTC); and
WHEREAS, the New Jersey Department of Health and Senior Services, has made grant funds available for the provision of such services at the Communicable Disease Prevention and Treatment Center; and
WHEREAS, on July 26, 2012, on behalf of the City of Newark, the Director of the Department of Child and Family Well-Being submitted an application to the New Jersey Department of Health and Senior Services, P.O. Box 360, Trenton, New Jersey 08625, requesting grant funding in the amount $138,693.00, for the continued provision of sexually transmitted diseases outreach educational services; and
