WHEREAS, the Department of Child and Family Well-Being provides childhood lead poisoning prevention services for children six (6) months through six (6) years of age under the Childhood Lead Poison Prevention Program; and
 
WHEREAS, the New Jersey, Department of Health and Senior Services has made grant funds available for the provision of such services through the Childhood Lead Poison Prevention Program; and
 
      WHEREAS, On June 9, 2012, on behalf of the City of Newark, the Director of the Department of Child and Family Well-Being submitted an application to the State of New Jersey, Department of Health and Senior Services, P. O. Box 364, Trenton, New Jersey 08525, requesting grant funding in the amount of four hundred eighty one thousand four hundred ninety three dollars and no cents ($481,493.00) for the Childhood Lead Poison Prevention Program, to provide lead poison prevention, community outreach and case management for children six (6) months through six (6) years of age; and
 
WHEREAS, as a result of this application, the City of Newark has been awarded a grant in the amount of four hundred eighty one thousand four hundred ninety three dollars and no cents ($481,493.00) from the State of New Jersey, Department of Health and Senior Services, Childhood Lead Poison Prevention Program, to provide lead poison prevention service, community outreach and case management for children six (6) months through six (6) years of age, for the period commencing on July 1, 2012 through June 30, 2013; and
 
WHEREAS, the City of Newark recognizes that the State of New Jersey, Department of Health and Senior Services, Childhood Lead Poison Prevention Program, may extend the time of the Grant Award and/or provide additional funds if made available; and
 
WHEREAS, the State of New Jersey, Department of Health and Senior Service, Childhood Lead Poison Prevention Program, will amend the grant to reflect the extension of the grant period and/or increase or decrease of additional funds if made available; and
 
WHEREAS, the City of Newark wishes to agree to any amendments to the grant.
 
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
    1. The Mayor and/or his designee, the Director of the Department of Child and Family Well-Being, are hereby authorized to submit an application to the State of New Jersey, Department of Health and Senior Services, P. O. Box 364, Trenton, New Jersey 08525, requesting grant funding in the amount of four hundred eighty one thousand four hundred ninety three dollars and no cents ($481,493.00), for the provision of lead poison prevention service, community outreach and case management for children six (6) months to six (6) years of age with no City funds required (a copy of the application is attached hereto and made a part hereof).
    2. The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.
    1. The grant award in the amount of four hundred eighty one thousand four hundred ninety three dollars and no cents ($481,493.00) from the State of New Jersey, Department of Health and Senior Services is hereby accepted (a copy of the award is attached hereto and made a part hereof).
    2. The Mayor and/or his designee, the Director of the Department of Child and Family Well-Being are hereby authorized to receive and expend the grant award from the State of New Jersey, Department of Health and Senior Services, Childhood Lead Poison Prevention Program, for the period commencing on July 1, 2012 and terminating on June 30, 2013 (a copy of the award letter is attached and made a part hereto).
    1. The Municipal Council hereby ratifies the acceptance of this grant award from July 1, 2012 to the date of adoption of this authorizing resolution.
    2. No Municipal funds are required for receipt or acceptance of this grant award.
    3. The Director of the Department of Child and Family Well-Being shall file a fully executed copy of the grant application and notice of grant award in the Office of the City Clerk.  
 
 
STATEMENT
 
This resolution ratifies and authorizes the actions taken by the Director of the Department of Child and Family Well-Being to apply for and accept grant funding from the State of New Jersey, Department of Health and Senior Services, P. O. Box 364, Trenton, New Jersey 08525, in the amount of four hundred eighty one thousand four hundred ninety three dollars and no cents ($481,493.00), for the provision of childhood lead poison prevention services, community outreach and case management for children six (6) months through six (6) years of age, for the period commencing on July 1, 2012 and terminating on June 30, 2013.  No municipal funds are required from the municipal budget.
