WHEREAS, the Department of Child and Family Well-Being, provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and
WHEREAS, the State of New Jersey Department of Health and Senior Services/ Vaccine Preventable Disease Program, has made funds available for the provision of such services; and
WHEREAS, on November 19, 2012, on behalf of the City of Newark, the Director of Child and Family Well-Being submitted an application to the State of New Jersey Department of Health and Senior Service/Vaccine Preventable Disease Program, P.O.
