WHEREAS, the City of Newark provides sexually transmitted diseases outreach and educational services to include, but not limited to high quality medical and laboratory services to residents in the City of Newark, requiring services at the Communicable Disease Prevention and Treatment Center (CDPTC); and
 
WHEREAS, the New Jersey Department of Health and Senior has made grant funds available for the provision of such services at the Communicable Disease Prevention and Treatment Center; and
 
WHEREAS, on July 24, 2013, on behalf of the City of Newark, the Director of the Department of Child and Family Well-Being submitted an application to the New Jersey Department of Health and Senior Services, P.O. Box 360, Trenton, New Jersey 08625, requesting grant funding in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), for the continued provision of sexually transmitted diseases outreach educational services; and
  WHEREAS, as a result of this application, the City of Newark has been awarded a grant in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00) from the New Jersey Department of Health and Senior Services, for the period commencing on July 1, 2013 and terminating on June 30, 2014; and
 
WHEREAS, the City of Newark, Department of Child and Family Well-Being desires to accept funds in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00); and
 
WHEREAS, the City of Newark recognizes that the New Jersey Department of Health and Senior Services may extend the time of the grant award and/or provide additional funds if made available; and
 
WHEREAS, the New Jersey Department of Health and Senior Services will amend the grant to reflect the grant time extension and/or increase or decrease of additional funds if made available; and
 
WHEREAS, The City of Newark, Department of Child and Family Well Being shall, upon any extension of time of the grant and or increase or decrease of additional funds, file an amending resolution reflecting the final time and award amount of the grant; and  
 
WHEREAS, terms of this grant award may only be waived or amended by the parties duly authorized to execute the grant, subject to the approval of the Newark Municipal Council by resolution.
 
 
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
    1. The Mayor and/or his designee, the Director of the Department of Child and Family Well-Being, are hereby authorized to submit an application to the  New Jersey Department of Health and Senior Services, P.O. Box 360, Trenton, New Jersey 08625, requesting funds in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), to provide sexually transmitted diseases outreach educational services to include, but not limited to high quality medical and laboratory services to residents in the City of Newark requiring services at the Communicable Disease Prevention and Treatment Center with no City funds required (a copy of the application is attached hereto and made a part hereof).
    2. The Municipal Council hereby ratifies its authorization of this application from the date of the application submission to the date of adoption of this authorizing resolution.
    1. The grant award in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), from the New Jersey  Department of Health and Senior Services, is hereby accepted ( A copy of the  award letter is attached hereto and made a part hereof).
    2. The Mayor and/or his designee, the Director of the Department of Child and Family Well-Being, are hereby authorized to receive and expend the grant award for the continued provision of sexually transmitted diseases outreach educational services for the period commencing on July 1, 2013 and terminating on June 30, 2014 (a copy of the award letter is attached hereto and made a part hereof).
    3. 5.      The City of Newark, Department of Child and Family Well Being shall, upon any extension of time of the grant and or increase or decrease of additional funds, file an amending resolution reflecting the final time and award amount of the grant.  
    1. 6.      The Municipal Council hereby ratifies the acceptance of this grant award from July 1, 2013 to date of adoption of this authorizing resolution.
      1. No municipal funds are required for receipt or acceptance of this grant award.
    1. The Director of Department of Child and Family Well-Being shall file a fully executed copy of the grant application and notice of grant award letter in the Office of City Clerk.
 
 
 
STATEMENT
 
This resolution ratifies and authorizes the actions taken by the Mayor and the Director of the Department of Child and Family Well-Being to apply for and accept grant funds in the amount of eighty nine thousand, one hundred thirteen dollars and zero cents ($89,113.00), from the New Jersey Department of Health and Senior Services, to provide sexually transmitted diseases outreach and educational services, for the period commencing on July 1, 2013, and terminating on June 30, 2014. No municipal funds are required.
