WHEREAS, the City of Newark's Department of Health and Community Wellness provides childhood lead poisoning prevention services for children six (6) months through six (6) years of age under the Childhood Lead Poison Prevention Program; and

WHEREAS, the New Jersey Department of Health and Senior Services, has made grant funds available for the provision of such services through the Childhood Lead Poison Prevention Program; and

WHEREAS, on April 12, 2016, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness, submitted an application to the New Jersey Department of Health and Senior Services, 50 State Street, Trenton, New Jersey 08525-0360, requesting grant funding in the amount of four hundred sixty-five thousand, seven hundred eighty dollars and zero cents ($465,780.00), for the Childhood Lead Poison Prevention Program, to provide lead poison prevention services, community outreach and case management for children six (6) months through six (6) years of age; and
