WHEREAS, the City of Newark desires to improve on clinical outcomes under the Heart Disease and Stroke Program Proposal by modifying, upgrading and the purchase of Practice Management/Electronic Medical Records (PM/EMR) software modules, which would allow for capturing data, establishing base lines and assessing health outcomes for residents of the City of Newark; and
WHEREAS, the New Jersey Department of Health and Senior Services has made grant funds available that can be used to modify, upgrade and purchase PM/EMR software modules; and
WHEREAS, on June 30, 2016, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness submitted an application to the New Jersey Department of Health and Senior Services, P.O.
