Additional Information:
On October 5, 2016, pursuant to Resolution 7R2-c, the DHCW accepted $12,000.00 from the DoAS. On October 31, 2017, the DoAS notified the City of Newark that it was awarding it no-cost extension dollars in the amount of $3,000.00, thus extending the contract period and bringing the total Program grant amount to $15,000.00.

WHEREAS, the New Jersey Department of Human Services, Division of Aging Services (DoAS), announced the availability of Falls Prevention Programs grant funds to increase the number of New Jersey's seniors and adults with disabilities at risk of falling who successfully complete Falls Prevention Programs; and
