WHEREAS, the Department of Health and Community Wellness, provides nursing case management/environmental investigation services for lead poisoned children, and promotes to the public, healthcare providers, and other professionals age-appropriate screenings, medical homes, and healthy/safe housing to the residents of the City of Newark; and

 

WHEREAS, the New Jersey Department of Health (NJDOH), Division of Family Health Services, has made grant funds available for the provision of such services through the Child Health 2019 Grant; and

 

WHEREAS, on May 22, 2018, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness submitted an application to NJDOH, requesting grant funding in the amount of $1,954,000.00 from the Child Health 2019 Grant to provide nursing case

management/environmental investigation services for lead poisoned children, and promote to the public, healthcare providers, and other professionals age-appropriate screenings, medical homes, and healthy/safe housing; and

 

WHEREAS, as a result of this application, the City of Newark has been awarded a grant in the amount of $1,954,000.00 from the NJDOH to provide nursing case management/environmental investigation services for lead poisoned children, promote to the public, healthcare providers, and other professionals age appropriate screenings, medical homes, and healthy/safe housing for the period commencing on July 1, 2018 and terminating on June 30, 2019; and

 

 WHEREAS, the City of Newark, Department of Health and Community Wellness, desires to accept funds in the amount of $1,954,000.00; and

 

WHEREAS, the City of Newark recognizes that the NJDOH may extend the time of the Grant Award and/or provide additional funds, if made available; and

 

WHEREAS, the NJDOH will amend the grant to reflect the extension of the grant period and/or increase or decrease of additional funds, if made available.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to submit an application to the New Jersey Department of Health, Division of Family Health Services, requesting grant funding in the amount of $1,954,000.00 from the Child Health 2019 Grant to provide nursing case management/environmental investigation services for lead poisoned children, and promote to the public, healthcare providers, and other professionals age-appropriate screenings, medical homes, and healthy/safe housing, with no City funds required (a copy of the application is attached hereto and made a part hereof).

 

2.                     The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.

 

3.                     The grant award in the amount of $1,954,000.00 from the NJDOH, Child Health 2019 Grant, is hereby accepted (a copy of the award is attached hereto and made a part hereof).

 

4.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to enter into and execute grant documents with the NJDOH to receive and expend the grant award in the amount of $1,954,000.00  to provide nursing case management/environmental investigation services for lead poisoned children, promote to the public, healthcare providers, and other professionals age appropriate screenings, medical homes and healthy/safe housing, for the period commencing on July 1, 2018 and terminating on June 30, 2019 (a copy of the Agreement is attached hereto and made a part hereof).

 

5.                     The agreement between the City of Newark and the NJDOH is hereby ratified from July 1, 2018 to the date of adoption of this authorizing resolution.

 

6.                     No municipal funds are required for receipt or acceptance of this grant award.

 

7.                     The Director of the Department of Health and Community Wellness shall file a fully executed copy of the grant application, the grant award letter, and the grant agreement with the Office of the City Clerk.

 

 

STATEMENT

 

This resolution ratifies and authorizes the Mayor and/or his designee, the Director of the Department of Health and Community Wellness, to apply for and accept grant funding in the amount of $1,954,000.00 from the New Jersey Department of Health, Child Health 2019 Grant, to provide nursing case management/environmental investigation services for lead poisoned children, promote to the public, healthcare providers, and other professionals age-appropriate screenings, medical homes and healthy/safe housing, for the period commencing on July 1, 2018 and terminating on June 30, 2019.  No municipal funds are required.
