WHEREAS, the Department of Health and Community Wellness provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and

 

WHEREAS, the New Jersey Department of Health (NJDOH), Vaccine Preventable Disease Program, has made funds available for the provision of such services through the Immunization 2019 Grant; and

 

WHEREAS, on May 1, 2018, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness, submitted an electronic application to the NJDOH, Vaccine Preventable Disease Program, for the Immunization 2019 Grant, requesting grant funding in the amount of Five Hundred Twenty-Three Thousand Dollars and Zero Cents ($523,000.00), to continue support to selected initiatives to increase immunization levels; and

 

WHEREAS, as a result of the application, the City of Newark has been awarded a grant in the amount of Five Hundred Twenty-Three Thousand Dollars and Zero Cents ($523,000.00), from the NJDOH, Vaccine Preventable Disease Program, to continue support to selected initiatives to increase immunization levels for the period commencing on July 1, 2018 and terminating on June 30, 2019 (a copy of NJDOH Notice of Grant Award is attached hereto and made a part hereof); and

 

WHEREAS, the City of Newark, Department of Health and Community Wellness, desires to accept funds in the amount of Five Hundred Twenty-Three Thousand Dollars and Zero Cents ($523,000.00); and

 

WHEREAS, the City of Newark recognizes that the NJDOH, Vaccine Preventable Disease Program, may extend the time of the Grant Award and/or provide additional funds, if made available; and

 

WHEREAS, the NJDOH, Vaccine Preventable Disease Program, will amend the grant to reflect the extension of the grant period and/or increase or decrease additional funds, if made available.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to submit an electronic application to the NJDOH, Vaccine Preventable Disease Program, for the Immunization 2019 Grant, requesting grant funding in the amount of Five Hundred Twenty-Three Thousand Dollars and Zero Cents ($523,000.00), to continue support to selected initiatives to increase immunization levels.

 

2.                     The Municipal Council hereby ratifies its authorization of this application from the date of application submission to the date of adoption of this authorizing resolution.

 

3.                     The grant award in the amount of Five Hundred Twenty-Three Thousand Dollars and Zero Cents ($523,000.00) from the NJDOH, Vaccine Preventable Disease Program, is hereby accepted.

 

4.                     The Mayor and/or his designee, the Director of the Department of Health and Community Wellness, are hereby authorized to enter into and execute grant documents with the NJDOH, Vaccine Preventable Disease Program, to receive and expend the grant award in the amount of Five Hundred Twenty-Three Thousand Dollars and Zero Cents ($523,000.00), to continue support to selected initiatives to increase immunization levels, for the period commencing on July 1, 2018 and terminating on June 30, 2019.

 

5.                     The Municipal Council hereby ratifies the acceptance of this grant award from July 1, 2018 to the date of adoption of this authorizing resolution.

 

6.                     No municipal funds are required for receipt or acceptance of this grant award.

 

7.                     The Director of the Department of Health and Community Wellness shall file a fully executed copy of the grant application, grant award letter and all supporting documents with the Office of the City Clerk.

 

 

STATEMENT

 

This resolution ratifies and authorizes the Mayor and or/his designee, the Director of the Department of Health and Community Wellness, to apply for and accept a grant award in the amount of Five Hundred Twenty-Three Thousand Dollars and Zero Cents ($523,000.00) from the NJDOH, Vaccine Preventable Disease Program, to continue support to selected initiatives to increase immunization levels, for the period commencing on July 1, 2018 and terminating on June 30, 2019. No municipal funds are required for receipt or acceptance of the grant award.
