Original Resolution 7R9-b adopted on June 6, 2019 for a contract amount of $150,000.00 to provide psychological services, including evaluations for fitness for duty, pre-employment examinations, re-evaluations, recruit screening, post-critical incident interventions, related testing, medical consultations and expert testimony for the Department of Public Safety, Divisions of Police and Fire.
The original contract declared that Pre-Employment Evaluations and Full-Time Recruit Screening as $375.00 per exam.
The First Amendment will correct the contract regarding the price of Pre-Employment Evaluations and Full-Time Recruit Screening to $500.00 per exam and the spelling of Dr. Jemour Maddux’s name on the contract signature page.

WHEREAS, the City of Newark, by Resolution 7R9-b adopted on June 6, 2019, awarded a contract to Lamb and Maddux L.LC. to provide Clinical and Forensic Psychological and Consulting Services from April 1, 2019 through March 31, 2020, for an amount not to exceed One Hundred Fifty Thousand Dollars and Zero Cents ($150,000.00); and
WHEREAS, the original contract with Lamb and Maddux L.L.C. requires an amendment to correct the price for pre-employment evaluations or full time recruitment screenings and the spelling of Dr. Jemour Maddux name on the signature page; and
WHEREAS, the contract previously read the price for pre-employment evaluations or full time recruit screenings was Three Hundred Seventy-Five Dollars and Zero Cents ($375.00), however, the correct price is Five Hundred Dollars and Zero Cents ($500.00); and
WHEREAS, the Director of Public Safety has authorized this amendment; and
WHEREAS, all other terms and conditions of the contract remain the same.
NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. The Mayor and/or his designee, the Director of Public Safety, are authorized to enter into and execute the First Amendment to the Agreement to provide Clinical and Forensic Psychological and Consulting Services with Lamb and Maddux L.L.C., 90 Main Street, Suite 101-1, Hackensack, New Jersey 07601 to correct the pricing for pre-employment evaluations or full time recruitment screenings and the spelling of Dr. Jemour Maddux name on the signature page.
2. The Municipal Council ratifies the contract amendment from April 1, 2019 to the date of adoption of this authorizing resolution.
3. All other terms and conditions of the contract shall remain the same.
4. A fully executed copy of the First Amendment to the Agreement to provide Clinical and Forensic Psychological and Consulting Services shall be filed in the Office of the City Clerk by the Director of Public Safety.
5. No further amendments or modifications to this amended contract shall be permitted without prior Municipal Council approval.
STATEMENT
This resolution ratifies and authorizes the Mayor and/or his designee, the Director of Public Safety, to enter into and execute the First Amendment to the Agreement to provide Clinical and Forensic Psychological and Consulting Services with Lamb and Maddux L.L.C., 90 Main Street, Suite 101-1, Hackensack, New Jersey 07601, to correct the price for pre-employment evaluations or full time recruitment screenings and the spelling of Dr. Jemour Maddux name on the signature page.
