Goal: Reduce and eliminate the incidence of vaccine-preventable diseases affecting children, adolescents, and adults through immunization. The Department of Health and Community Wellness will offer a variety of immunization services to community residents' ages 0 through 18 years.

WHEREAS, the Department of Health and Community Wellness provides immunization services to increase immunization coverage levels for children and adults through the Immunization Action Plan/Project Vaccinate Program in the City of Newark; and
WHEREAS, the State of New Jersey Department of Health - Vaccine Preventable Disease Program has made funds available for the provision of such services; and
WHEREAS, on May 20, 2020, on behalf of the City of Newark, the Director of the Department of Health and Community Wellness (the "City"), submitted an application to the State of New Jersey Department of Health - Vaccine Preventable Disease Program (Immunization 2021), P.O.
