WHEREAS, the City provides trauma support and services to those who have been impacted by trauma and victimization; and                     

 

WHEREAS, on or about September 15, 2021, University Hospital, 150 Bergen Street, Newark, New Jersey 07101, proposed collaboration with the City of Newark (the “City”) for the City to provide Community Health Worker (CHW) services for University Hospital’s Hospital-Based Violence Intervention Program (HVIP); and

 

WHEREAS, University Hospital made grant funds available for the provision of CHW services to provide case management and care coordination services to HVIP participants; and

 

WHEREAS, the City and University Hospital desire to enter into a Professional Services Agreement and associated Business Associate Agreement,  setting forth the terms and conditions for the inclusion of the City in the Hospital’s HVIP Program;

 

WHEREAS, on or about April 28, 2022, University Hospital awarded the City a grant in the amount of Sixty Thousand One Hundred Sixty Dollars and Zero Cents ($60,160.00), for the period of October 1, 2021 through September 30, 2022 (a copy of the Notice of Award is attached hereto and made a part hereof) to fund the City’s for the provision of CHW services for the HVIP at University Hospital; and

 

WHEREAS, the City desires to execute a Professional Services Agreement and associated Business Associate Agreement for the City to accept the grant in the amount of Sixty Thousand One Hundred Sixty Dollars and Zero Cents ($60,160.00), for the period of October 1, 2021 through September 30, 2022, to provide Community Health Worker (CHW) services for University Hospital’s Hospital-Based Violence Intervention Program (HVIP); and

 

WHEREAS, University Hospital may amend the grant to reflect the grant time extension and/or increase or decrease of additional funds if made available; and

 

WHEREAS, the Office of Violence Prevention and Trauma Recovery shall, upon any such extension of time of the grant and/or increase or decrease of additional funds, cause a budget insertion to be submitted to the Municipal Council for its consideration in the amount of the grant; and 

 

WHEREAS, terms of this grant award may only be waived or amended by the parties duly authorized to execute the grant, subject to the approval of the Municipal Council by Resolution.

 

NOW, THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:

 

1.                     The Mayor of the City of Newark and/or his designee, the Business Administrator, and the Director of the Office of Violence Prevention and Trauma Recovery, are hereby authorized to accept grant funds in the amount of Sixty Thousand One Hundred Sixty Dollars and Zero Cents ($60,160.00) and execute a Professional Services Agreement and associated Business Associate Agreement, from University Hospital, to provide Community Health Worker (CHW) services for University Hospital’s Hospital-Based Violence Intervention Program (HVIP).

 

2.                     The resulting grant award in the amount of Sixty Thousand One Hundred Sixty Dollars and Zero Cents ($60,160.00), from University Hospital, is hereby accepted.

 

3.                     The Municipal Council hereby ratifies the acceptance of this grant award from October 1, 2021 to the date of adoption of this authorizing Resolution.

 

4.                     The Mayor of the City of Newark and/or his designee, the Business Administrator, and the Director of the Office of Violence Prevention and Trauma Recovery, are hereby authorized to receive and expend the grant to award to provide CHW services for the HVIP at University Hospital, at 150 Bergen Street, Newark, New Jersey 07101, for the period commencing on October 1, 2021 and ending September 30, 2022.

 

7.                     The City shall, upon any extension of time of the grant, and/or increase or decrease of additional funds, cause a budget insertion to be submitted to the Municipal Council for its consideration.

 

8.                     No municipal matching funds are required for the receipt or acceptance of this grant award.

 

 

STATEMENT

 

This Resolution ratifies and authorizes the actions taken by the Mayor of the City of Newark and/or his designee, the Business Administrator, and the Director of the Office of Violence Prevention and Trauma Recovery, to accept and expend grant funds in the amount of $60,160.00 and to enter into and execute a Professional Services Agreement and associated Business Associate Agreement, from University Hospital, 150 Bergen Street, Newark New Jersey 07101, for the City of Newark to provide Community Health Worker (CHW) services for University Hospital’s Hospital-Based Violence Intervention Program (HVIP), for the period commencing on October 1, 2021 and ending September 30, 2022. No municipal matching funds are required for the receipt or acceptance of this grant award.
