WHEREAS, LaGuanda Frazier is a Certified Municipal Registrar (CMR); and,
WHEREAS, LaGuanda Frazier has served as the Registrar for several years, and is currently the Registrar with the City’s Department of Health and Community Wellness, Bureau of Vital Statistics; and,
WHEREAS, the City of Newark is required to take action to formally recognize the re-appointment of the Certified Municipal Registrar; and
NOW THEREFORE, BE IT RESOLVED BY THE MUNICIPAL COUNCIL OF THE CITY OF NEWARK, NEW JERSEY, THAT:
1. Pursuant to N.J.S.A. Title 26, Chapter 8 et seq., La Guanda Frazier is hereby re-appointed as the Certified Municipal Registrar, commencing on July 1, 2024, for a period of three (3) years ending on June 30, 2027, during which time period the Appointee shall be obligated to continue to satisfy all requirements of the New Jersey Department of Health (DOH), Office of Vital Statistics and Registry (OVSR).
2. This appointment is hereby ratified to July 1, 2024, to the date of this authorizing Resolution.
3. A copy of this Resolution will be sent to the New Jersey Department of Health, Office of Vital Statistics.
STATEMENT
Resolution re-appointing LaGuanda Frazier, as the Certified Municipal Registrar (CMR), for the City of Newark, Department of Health and Community Wellness, Bureau of Vital Statistics, commencing on July 1, 2024, for a period of three (3) years ending on June 30, 2027 and/or in perpetuity till a succeeding appointment is determined in conformance with all applicable laws and statutes of the State of New Jersey.
